Gynecomastia is a condition in a male patient where an overdevelopment of the pectoral gland or excessive fatty tissue in or around the gland causes the male breasts to appear excessively large and/or droopy. Gynecomastia is a medical term coined from the Greek expression for "woman-like breast." Though this oddly named condition is rarely mentioned, it's quite common, affecting, to a various degree, 40 to 60 percent of male population.
Gynecomastia may affect only one breast or both. Although, in vast majority of cases, there is no known cause for Gynecomastia, there are some known factors that contribute to this condition. These include hormonal treatments (such as anabolic steroids) administered at puberty, or even in adulthood, obesity, alcohol and marijuana.
Dr. Chugay makes a small incision in the region of the areola and removes the entirety of pectoral gland tissues on both sides. After the gland is removed, meticulous microlipusculpture is conducted of the whole chest region.
If there is excess skin present, the skin is removed in a doughnut pattern around the areola; the skin margins are widely undermined, and a permanent purse string stitch is placed underneath the skin.
The fullness of the breast is corrected by removal of the pectoral gland.
The unsightly irregularities, common for procedures that involve the removal of the pectoral gland without skin tightening or contour correction, are avoided by simultaneous liposculpture of the entire chest region and the removal of the overhanging excess skin.
The utilization of permanent stitch will cause the scar around the areola to become almost imperceptible within four to five months.
The chest becomes firmer, flatter, more even and better contoured.
Dr. Chugay feels that pectoral gland excision must accompany the correction of any case of substantial Gynecomastia. Many surgeons still treat Gynecomastia with liposuction only, thus neglecting to remove very strong fibrous pectoral gland tissues, which appreciably contribute to the excessive fullness of male chest.
This procedure, which involves the use of external ultrasound and specially-shaped small microcannulas, helps to even out the areas of depression and unevenness, thus producing even exterior of the pectoral region. READ MORE ABOUT EXTERNAL ULTRASOUND.
This technique allows for inconspicuous removal of the excessive skin around the areola without producing large visible scars. It is important to remember that significant scarring usually accompanies the more aggressive classic types of skin reduction procedures. The scar, realized from the purse string permanent suture, will normally become unnoticeable within four to five months. You can learn more about this technique in the BREAST LIFT article.
Surgery may be discouraged for obese men, or for overweight men who have not first attempted to correct the problem with exercise or weight loss. Also, individuals who drink alcohol beverages in excess or smoke marijuana are usually not considered good candidates for surgery. These drugs, along with anabolic steroids, may cause gynecomastia. Therefore, patients are first directed to stop the use of these drugs to see if the breast fullness will diminish before surgery is considered an option.
When a qualified surgeon performs Gynecomastia correction surgery, complications are infrequent and usually minor. Nevertheless, as with any surgery, there are risks. These include infection, skin injury, excessive bleeding, adverse reaction to anesthesia, and excessive fluid loss or accumulation. The procedure may also result in noticeable scars, permanent pigment changes in the breast area, or slightly mismatched breasts or nipples. If asymmetry is significant, a second procedure may be performed to remove additional tissue.
The temporary effects of breast reduction include loss of breast sensation or numbness, which may last up to a year.
You'll be swollen and bruised for a while--in fact, you may wonder if there's been any improvement at all. To help reduce swelling, you'll be instructed to wear an elastic pressure garment continuously for a couple of weeks, and for a few weeks longer at night. Although the worst of your swelling will dissipate in the first few weeks, it may be three months or more before the final results of your surgery are apparent. The final results of the procedure are significant and permanent. If your expectations are realistic, chances are good that you'll be very satisfied with your new look.
Dr. Chugay utilizes a combination of local anesthesia and IV sedation. You will be asleep and comfortable during the procedure. The risky general anesthesia is not used.
